IMPORTANT NOTICES:

·        The deadline for appointments for the

·        February 22^nd, 2008 Convention is  February 6, 2008 . This deadline will be strictly enforced.

·        Please enclose the appropriate annual dues payment with your form: Associate (Over Age 30): $16 

     Associate (Age 30 and Below): $12   

     Youth Associate (Under 18): $6

In accordance with sec. 2.01.08 (C), This appointment form must be accompanied with payment of the appropriate dues in order to be processed and the appointments made herein to be valid.
